What is it?

A tooth brush add showing the new aquafresh flexigel toothbrush. It shows the tooth brush going into this guys mouth and coming out of his ear.

What was used to create it?

Photoshop was used to create this by simply lassoing the top part of the tooth brush, turning it sideways, and having it stick out of his ear.
